Assist — Life Stage & Transition

Big transitions are where support matters most and is often hardest to find. Leaving school, moving out, changing providers, entering work, losing a carer — we walk through those chapters with you.

We support planning and practical navigation through school-to-work transitions, moving into independent or supported living, changes in health or capability, family changes including the loss of a carer, and shifts between service systems.

Transitions are as much emotional as logistical. Alongside the paperwork and coordination, we make room for the fact that change is unsettling — for the participant and for the family around them.

We also plan ahead where we can. Many transitions are foreseeable, and the difference between a planned move and a crisis move is enormous. If something is coming, we'd rather start early.

Earlier than feels necessary. Planned transitions go far better than crisis ones — if something is on the horizon, let's start now.
Yes. Transitions affect everyone around the participant, and we make space for that rather than treating it as a purely logistical exercise.
Usually under Capacity Building — Improved Relationships or Improved Daily Living, depending on the transition.
